Filling out the PTR

Greetings SA forum members. I have a quick question about the PTR, and I was hoping that someone who has filled it out can provide an answer...

The PTR checklist lists two items, the "Immunization Record" and the "Mantoux Tuberculin Skin Test (PPD) form" as two separate documents.

From what I have seen, the PPD test is included on the Immunization Record... Am I correct, or is there a separate "PPD form" that I am missing?
Should I have an Immunization Record sheet and an additional document for the PPD test, or are they included on the same sheet?

I would hate to miss anything or leave any forms out...

Good question. My DS has been wondering about the same thing. In the booklet of instructions, it says something about document showing the results of the PPD test. I think you just need to request something from your physician showing the results. My DS is picking up his completed Immunization form tomorrow and I told him to also ask for some document showing the results of the PPD test. I'll report back tomorrow to see if he gets it no problem.
 
When I went to pick up my Immunization Record sheet, the doctor wrote the results of the PPD test on the corresponding section of the green sheet... I am hoping that will be sufficient proof of the results of the test.

My DS did, in fact, get a separate certificate of the results of his PPD test. They do explicitly request that ND. I'd consider calling your physician's office and requesting such a certificate.
 
You can print a standard form. Just search for "Tuberculin Skin Test Standard Form". We printed one out and had the Dr. just transcribe the info. I believe it does have to be documented on a separate form giving the Lot # of the vial as well as date "placed" and date "read". Hope that helps.
 
And the news is that they will be re-tested once they get to the academy. Rational, the Mid/Cadet may have been exposed between the time of the latest test and reporting in to the Academy.
Some insurance carriers may not authorize payment for certain immunizations. The Mid/Cadet will receive those once they go through medical (sore arms and feeling a bit cruddy for some, that is the down side).
 
TB is re-appearing in some schools, so I would recommend having the Mantoux re-done; it is not an expensive test and can be done at your community's public health office.

My son's doctor also just indicated the test results on the immunization record? I could not find the actual form by googling for tuberculin skin test standard form? Would anyone have a link by chance?
