Fleet Hometown News Release (missing page)

DD is wrapping up the final paperwork here, and just found that the paper copy she received of the "Fleet Hometown News Release" --NAVSO 5741/1 (Rev 4/2012) -- is printed on one side and blank on the other (the bottom footer saying "Page 1 of 2").

Unless we missed something, the form is not on the USNA Permit to Report link (please advise if we missed something!). We have also tried finding a public-domain copy to use as a substitute, but to no avail.

Any ideas?

I know this isn't critical processing info, but we would rather get this all into one envelope, and have it arrive on-time. Thanks!

News release doesn't go back to USNA anyway. It's a template for news outlets to use to announce your candidate's acceptance if you choose to send one to them.

The Permit to Report package contains a separate envelope addressed to the Dean of Admissions at USNA (with the red-marker annotation for "PAO," which we understand to be the Public Affairs Office), and it has a May 24 deadline. DD is just trying to stay within the instruction limits, and the missing form page is frustrating that effort.
 
DO NOT send it to the points of contact on page 2 of the FHTN form. The USNA PAO specifically wants to collect all of them and disseminate it through the FHTN office.
