Hi all,

In completing USAFA form 147 (Candidate Activities Record), when filling out the resume section of the form, I accidentally deleted the instructions for what information they want. I have completed it as well as I can without the exact instructions for each category. Does anybody know exactly what they are asking for? (Particularly for the “Academic Accomplishments” and “Leadership and Extracurricular Activities” sections).

ACADEMIC ACCOMPLISHMENTS (9th grade to present)

List your academic accomplishments, advanced courses, college level, AP & IB, Honors courses to include classes taken after high school and/or while enlisted.

LEADERSHIP & EXTRACURRICULAR ACTIVITIES (10th grade to present)
===============================================================
Describe all leadership positions you have held and extracurricular activities you were involved in to include athletic activities. Please include activities or leadership positions held during college and/or while enlisted.


COMMUNITY INVOLVEMENT (10th grade to present)
=============================================
Detail all community activities/volunteer work you participated in and how you helped others. What were your responsibilities? Please include any activities or volunteer work that you did in college and /or while enlisted.



EMPLOYMENT HISTORY (10th grade to present)

List all employment you have held, part-time and/or full-time while in school and during summer. List dates of employment and what you did. Please include any work done while in college and/or while enlisted.



AWARDS AND ACCOMPLISHMENTS (10th grade to present)
==================================================
List any awards or noted accomplishments. Give dates when received. Please include any of these received while in college and/or while enlisted. EX: National/State awards, Scholarships, Distinguished Graduate, etc.

Sorry, at work and pulled up my USAFA application from my phone to do that. Formatting might be a little rough but those are the instructions for each section. 👍🏻👍🏻
