Forms for R-Day

Hey everyone i was just wondering what forms I will need to bring with me on R-Day? I know it says to bring the direct deposit form but I just figured I'd ask what other ones are important (like birth certificates, immunization records, etc.??)??

I would hate to show up on R-Day without some forms that are required! So any input on which ones I should bring would be appreciated. Thanks a lot!!

Dont forget a copy of your Social Security Card

Maj Belmont's assistant Ms. Rush told me that copies of my Birth Certificate, SS Card, and Immunization Forms will be sufficient, provided that I had mailed in all the other required documents.
