Help: Where to find certain forms to submit to West Point Academy

Hello, my Second Step Kit was opened on June 25th, and my main setback has been my inability to find certain forms that I seem to need to download in order to sign them, do what I need, etc. And then Submit. Those being: the body fact worksheet, certificate of authorization, direct deposit form, dental screening form, employer evaluation, legal paperwork, parental consent form, tattoo form, and the vision survey form.
And as for my AP scores, would those be the scores of my AP classes? Or are those something else?

You don't need to upload everything. Forms will be provided to you if you are required to.
AP scores are AP Exam results from the college board.
 
@NJ12 many of the forms you describe were only made available to my class of 2025 DS within the last 6 weeks.

the portal will only let you submit those forms that USMA requires at a point in time. Direct deposit, tattoo etc are only required after acceptance of an appointment

I suppose I have an issue there. because I do not have any AP scores on my college board.

I am not the class of 2025, but the class of 2026.
 
Precisely my point. If my DS who is a year ahead of you only gained access to some of these forms recently- you don’t have to worry about it

Ahh I see, thank you for clarifying. Though the problem is I have messaged the SSK on the 25th telling me I had to get all the documents submitted within 60 days.

Not a huge problem, but just trying to save you from getting roasted by anyone in the future.

Please don't refer to USMA as "West Point Academy."

It's the United States Military Academy at West Point, but most people just say West Point or USMA.
 
There are links in your portal to every form you need to submit, both during the application process and after. As mentioned above, some of the forms you referenced are not required until/unless you receive an offer of admission and accept.
