history of anxiety disorder

How does the DOMERB review board determine a "history of anxiety disorder" and possible disqualification if that diagnosis was not listed in the medical history form filled out by my daughter? I believe I need to ask the question here in order for my daughter to be able to email Mr. Mullen and ask for help / advice?
