How to Submit CFA, Academic Info/Evaluation, etc?

Once one takes the CFA, do the results need to be emailed to admissions or should it be submitted via the online application or elsewhere? The document upload section only gives the option for the essay and resume. Additionally, how would one get the required forms for academic information and evaluations?

Submit to whom/where?
Candidate Fitness Assessment (CFA):

You must pass the CFA, an evaluation of your physical fitness, before your name will be forwarded to DoDMERB to schedule your medical examination. The CFA may be administered by a Physical Education Teacher, J/ROTC instructor, Academy Field Representative, or any commissioned/non-commissioned officer other than a parent/guardian. The CFA should be submitted after June 1st. Scores for a CFA administered before June 1st will not be accepted.

a. Enter the Last Name, First Name, Email Address and Phone Number of the person that will administer the CFA. Ensure that the email address and phone number you provide is valid and current.If the phone number you provide for your contact is incorrect, they will not be able to access the online form.

b. After you submit the request, the designated point of contact will receive a secure email after 4 hours with instructions to administer the CFA and submit the results. You can update your point of contact information within the 4 hour window before the email is generated.

c. Your official CFA scores must be submitted directly to the Academy through the online form received by your designated point of contact. Instructions for administering the CFA can be accessed here: Candidate Fitness Assessment Instructions. d. When you have submitted the CFA request to your designated point of contact, your portal will update from “Not Submitted” to “Requested”. Once you point of contact submits the official scores, the portal status will be updated to “Verified Complete”.

e. Once submitted, your status page will be updated from “None” to either “Pass” or “Fail” in the CFA line.

f. You are authorized to take the CFA twice. If you fail one event, you must retake the entire CFA. Your electronic CFA form will be reset if you fail your first attempt. Candidates who fail the CFA after their second attempt will be disqualified from consideration.

g. If you have completed the CFA for another service academy, you can request the results be emailed to admissions@usmma.edu.
