does anyone know if an appointee can send in the immunization form incomplete (by the 5/15 deadline) then add missing vaccinations and bring a completed form to IDay?

DS has been away at college and couldn't get the local health clinic to fill in the form with info from home doc's office. Will be back and will get all the missing shots next week. Should he send in the form now, with stuff missing? Or send it a few days late, complete? (i.e.. add new info to a photo copy of what was sent)

The instructions make it clear that the "Admissions office will not be able to help", and parents/students are not to call the clinic directly.

I would do as you suggest; send in now with brief explanation attached and resend later with completed boosters etc. USMA would accept the form manually on R Day but I got it all done last year ahead of time. They wont kick you out for this; send it in when you get it done and also bring complete records with you on I day.

I was pulling my hair out last year trying to get DS' blood type; cannot believe how hard it is to actually get that done quickly if you don't already have a Red Cross donor card. Had to finally have pediatricians office give me an Rx for DS to go to hospital lab clinic down the road and get a blood draw for type test. And of course it would have been fine to show up on R day without that but why take chances.
 
Not sure about Navy, my DS is AF. But I can make a recommendation because we have had the same issues. My DS Doctor doesn't seem to care what Air Force or DOD requires. They are refusing to give the shots period because he has already had them. However, DOD requires certain shots to be administered within 6 months of Admission. Tell him to go this weekend to a local CVS or Walgreens that has a Minute Clinic and they will give him the shots (Or something in that area pharmacy type some of them will take certain insurances, ours was not accepted). Call first because some locations don't always have certain things in stock like Chicken pox or Polio ect.. we had to go to like 2 or 3 places to get all of the shots. and we are still waiting for results from a blood titer to see if he needs one more shot. Of course, I am paying out the nose for these shots. We paid over $300.00 when we could have had insurance cover them for free under our plan. Ugh

You and your son should know that if there are issues with any part immunization paperwork then they will just give him another immunization on the spot....up to and including the ALL of the vaccines listed.

They didn't like something that was on my friends record so they just re administered like 5 vaccines to him on I day.

DS has been trying to get an answer from Admissions. If they say it's ok to send it late but complete, that would be best. If he doesn't hear back, he'll send it incomplete, then have the Dr. add new info to the copy of what was sent and bring a copy on I-Day.
The directions say "ensure you get all required vaccinations", but later it says "any vaccines not received will be provided on I-Day at no cost".
Probably a good idea to avoid a bunch of shots on IDay if possible
 
I would think so, we have been doing them all different times of the week because my DS is an athlete in season and moving on into State Qualifying. Sucks to have sore arms in his sport, been a challenge this week. I can't imagine having a ton of shots on IDay and moving on into Basic. I mean it's doable, but certainly not preferred.

Sore spot with me when doctors offices treat you like you work for them. There was a thread last year on another forum about a doctor [not] releasing therapy records on request [when the 'request' was really an instruction to be followed.] Walk out, never go back. Find another doctor who treats you like a valued patient and client. On the bright side you save a lot on tuition next 4 years . . .
 
Midwest Dad, trust me its been really frustrating not only for my me, but my DS is about to lose his mind. I just told him listen, you have to be persistent with them not rude. Do you want to be an officer? He is your chance to prove you handle yourself like an officer. You simply say.. Ma'am or Sir I am required to have these shots or blood titers proving that I am immune before XXX Date. I have RX from the Air Force Academy for the said shots should the blood titers prove no immunity. Of course, our insurance and the Dr want to bill insurance for the visit of course when we called since it is a "well visit" they could get him into the office in like 45 days

Unfortunately, we don't get to pick insurance plan since it's through my DH work. I can switch Docs, but they are all the same way with the rules. Crazy right?

I recommend wait till it is completed to send in if it is only going to be a week late. Not every candidate received PTR in April. It is reasonable to wait till it’s done and send in so all shots are on it. They are not creating his medical record on 16 May.
 
I would call (not sure if previous inquiries only involved email) Admissions (or the USNA medical number, if provided) and ask. It might be more worthwhile to delay submission for a week than to have multiple forms floating around.

One idea is to request a ‘nurses’ visit. At least that’s what our clinic does. I get what you are saying about the well check, and only allowed once every so often. Our clinic however isn’t billing for a visit to DR. Rather only the shots. He isn’t seeing the dr. Same with TB test. DR reads and signs off but DS isn’t actually seeing him for an office visit.
