Information accuracy

I know that it is always preferable to be accurate, but some information that they request on the form is something I don't know and don't have the resources to find out (dates of diagnoses' and/or last treatments, etc.), but I can't leave those sections blank, what should I do?
 
Ask your parents? If you do not know if you have ever been diagnosed with a specific disorder then check 'no'.
If you have a regular physician/health care provider then call them and ask them to look at your records.
