Could anyone post the immunizations list that you have to have prior to reporting for the service academies. also elaborate on the dental.
 
I can only speak to what is required for USMA. These may be different for the other academies.

This is right off of the Immunization Record Form:
A. Tetanus and Ditphtheria (Td, TD, DT, DPT) – 4 doses plus 1 booster between the ages of 11 and 16.
B. Polio Virus Vaccine (OPV, IPV) – 4 doses plus 1 adult booster after the age of 18
C. Measles, mumps and rubella (MMR) – 2 doses, one after 1 year of age and the second must be at least 1 month after the first dose.
D. Hepatitis A Vaccine (Hep A) – 2 doses, second one must be at least 6 months after first dose. At least the first dose of the series is required for admission.
E. Hepatitis B Vaccine (Hep B) – 3 doses, second one must be at least one month after the first and third one must be at least 6 months after first dose. Series must be complete prior to admission.
F. Twin RX, Hep A and Hep B Combination Vaccine – 3 doses, second one must be at least one month after the first and third one must be at least 6 months after first dose. At least the first dose of the series is required for admission. (Not necessary if you have received/started the independent Hep A series and/or Hep B series.)
G. Varicella (Chicken Pox) – 2 doses, second must be at least one month later if vaccine was received as an adult. If you have had the chicken pox you must attach the results of a varicella titer to show proof of immunity.
H. Meningococcal (MGC, Menomune) Vaccine – 1 dose
I. Tuberculosis Test (IPPD) – required within 6 months of entrance into the USMA (prior to arrival).
J. Blood type – must attach proof of blood type.


For dental they send a form for the dentist to fill out, plus they require a panographic x-ray and bite-wing x-rays both done within the last 12 months.
