No. I don't think there is a question on the medical history exam that would even trigger you having to report a yeast infection. ( Unless it required hospitalization). An active yeast infection during your DoDMERB exam "may" be disqualifying, but I don't think that would even DQ.
 
No and you should only answer what they ask in the questionaire. Nothing more. They won't check for that either.

Just an FYI, although these isn't anything on the physical form that would appear to require disclosure of a yeast infection, there is a GYN questionnaire that can be triggered as a remedial request. I've seen that form, and it would could require disclosure of a yeast infection, depending on details of history and treatment. Very doubtful disclosure would be disqualifying without additional complicating factors.
