Marine Option NROTC Supplemental Forms and DoDMERB

Hello, all. I have a couple questions regarding the Marine-option NROTC scholarship process. I've finished the online application but have not submitted because I want to review them with my OSO. It also says I have to complete three "supplemental forms" that are "available on web site". The NROTC website is also extremely vague as to what these forms actually are, but I think I've found them:
1) Statement of Understanding
2) Debarment Understanding
3) Drug Statement
If there is anyone that has been through this process can verify that these are three correct forms?
Other question regarding the DoDMERB: I was medically qualified for USNA but I haven' seen anything regarding the medical exam for NROTC. Will they request my DoDMERB results or do I need to take action on that?
 
When I completed my app (Not a MO, just NROTC) I signed all those forms at my interview.
 
The forms you mentioned state "waiting to be processed" on the application. My son filled them out at the interview. Does that mean they are not ready to go before the board.

There is only one DoDMERB exam; if you are offered a scholarship, they send a request to DoDMERB and the results are sent to them. Last year, my son had the medical ordered by USNA. He passed their exam, needed an eyesight waiver for USCGA, and was ultimately offered a scholarship with AFROTC. No further action was required from him. If they have different standards than the USNA, NROTC will let you know.
 
They are not DoDMERB forms but NROTC forms. My DS filled out the forms and emailed them to his interviewing officer per his specific instructions. He also brought a copies to his interview, and as you can imagine, they did not have the emailed forms in his file, so he gave them the copies. You should bring a copy of all the NROTC forms with you to the interview, just in case. Also a copy of your resume.
