Medical Disqualification?

I just got my results back from DODMERB about my physical, everythings in order except they're making a remedial request for medical/surgical documentation about my Patelar tendinitis. I had an very mild case of tendinitis in my right knee when i ran cross country my freshman year of HS. it's now the summer after my senior year and I haven't had problems with the knee since freshman year. However when I was filling out the DODMERB medical questionnaire I put that I had to use orthodics for it and that it happened, I never officially went to see a doctor about it and was given the"diagnosis" by a physical therapist and a chiropractor. I'm worried this would be grounds for medical disqualification. Am I just being paranoid? I went to my regular doctor today and got a letter from them stating that all is well and I have no more problems with the knee and sent that to DODMERB. The letter also cited my multiple seasons in multiple sports without a problem
 
This is a good example on what to report on a DODMERB physical. Do not diagnose yourself. If you have not been diagnosed by a medical doctor then do not report it!
