Do I need to be medically qualified by DODMERB to participate in AFROTC if I am not on a scholarship?
From what I know, you need to be free of any medical issues that can prevent you from serving as if you were actually enlisted. Issues such as uncorrectable vision, severe joint problems, drug dependencies, etc. I'm a HS senior so I can't elaborate any further, I haven't considered AFROTC into much detail myself.

WRONG and RIGHT.
~ There is no physical exam required to join AFROTC. However, sure if your vision is so bad that your lenses are coke bottle bottoms, or you can't get through a day, month, yr without smoking pot, than don't join because the DoDMERB exam will be in your future as a cadet, even without a scholarship. The pot issue will also be part of your TS clearance...there are cadets that got dis-enrolled bc they lied about their mj use prior to college, and came clean later on.

If you are a walk on than NO, you do not need a DoDMERB exam as a freshmen. You just go to orientation, tell your academic advisor that you plan to join AFROTC. The school will schedule LLAB into your curriculum. ROTC will be informed during the summer and reach out to you about when you need to show up for AFROTC
~ Some schools will bring ROTC cadets in prior to the start of the school yr. Some will not. However, be prepared your 1st image in the unit is usually their 1st PT within the 1st week. Hence, work out all summer long so you are ready to do the PFA.

Now if you have done the DoDMERB exam, and did not receive a scholarship or a waiver, get in front of that now. The reason why is that to commission out of AFROTC you must attend Summer Field Training (SFT) in the summer of your rising junior yr. You will be sent for a medical exam prior to going to SFT if you are non-scholarship.
~ DoDMERB is only valid for 2 yrs. Some issues move from dq to waiver due to how far out it is from the original diagnosis. IE if you have been diagnosed with ADD and take prescribed meds for the last 24 mos., that is a strong DQ, no waiver. However, if you have been off meds, maintained the grades, for the last 24 months, than it is waiverable.
~~ If you took that exam in Aug 2018, they will make you take a new one as an AS200 prior to SFT since the exam will be older than 2 yrs.

You will also do a DoDMERB exam prior to commissioning. Hence, sooner or later DoDMERB will catch up to you.
 
Regardless of the ROTC program, although you do not need to complete DoDMERB out of the gate, you will need to pass it before you contract. All service require you to contract no later than the end of your sophomore year, or the start of your junior year; so as Pima says, it will be in your future, .
