What do we upload to the medical forms exactly? The whole form including vaccination record or just the cover part that lists the Doctor's Office info? Thank you!

DS uploaded what was sent to him by WP, his medical records that showed what vaccines he had received previously, and a copy of 2 recent vaccines he had received which were polio and meningitis. His vaccine portion in the portal just turned green.
I brought my immunization form to my physician and had them fill it out and provide me a copy with my records (including the polio booster we need for the Army). I checked over everything to ensure they correctly filled it all out (they didn't the first time, and accidently said I had TB on the screening form; crisis averted 
), then I scanned everything to a pdf and compressed it using some free pdf compressor and uploaded it to my portal. My portal still is not green, and I heard others saying the medical staff couldn't access it on the portal and to just send your pdf to the email on form 40-1. Hope this helps.
