Medical History fill-outs

I am trying to complete these medical history forms but when I begin, there are no boxes to check "yes" or "no." I have downloaded Adobe Reader, and still no help. Hope the picture helps.

Your best help will come from Concorde. My son had no trouble with this form but has had with others. Go to the source for assistance.
