Long time listener, first time caller. My DD is trying to accurately answer all the questions asked on her Medical History Survey ahead of her exam. There are a number of "Follow-Up Questions" that are near impossible to answer. For example, if your child had some "wheezing" (breathing issues) as a child, it asks how many occurrences and the exact dates. No clue how many times and when. It was at least 10-15 years ago. Unless there was a documented doctor's visit, how does she answer these questions accurately? It's not like we kept a log. I would greatly appreciate any guidance here. Thank you!
