medical records format?

My DD plans on applying to SAs and ROTC in 2020. Over this winter break I will be suggesting to her to begin getting her medical records in order. I suppose once she starts the process she'll get the answer to this, but in what format should she request the records? digital? CD? paper?

The records should be a pdf attachment or if under 20 pages, may be faxed to 719-333-3578
 
Your DD won’t need ‘alll her medical records’ as part of the normal application to a SA or ROTC program, if that’s what you are thinking. She will answer a questionnaire, which, along with her medical DoDMERB exam, MAY result in additional medical information requested (AMI’s). Which may or may not require the actual DR medical record (both of my ds’s amis were simply a form signed by DR). No need for her to contact all her DR’s and request her records for this process.

It is a good idea for her (and you) to make sure alll her ‘open’ items with her DR’s are resolved. For example, DS was never released to full participation in writing after an injury since he was verbally over the phone . So he needed to get that written authorization from his DR. Tying up those kinds of loose ends can be done, but y’all don’t need to contact all your DR’s and get copies of records (unless you want them for your own personal info) for this SA/NROTC process in general.
