Medical Status says qualified but has 2 things under it. Had a Labrum tear repaired and had Growth Hormone when he was younger. Not sure what to do for the AMI on the first one. So I am assuming we just send in information for the 2nd one which seems easy. Why does it say Qualified but need the stuff below?

R130.00 Provide all medical records regarding short stature and human growth hormone. Please submit any specialist's notes and results of any testing, most importantly any final laboratory testing showing resolution.
