Mistake on Medical History can't change it to no

So I marked some answers as yes on the medical history questionnaire and do not want to electronically submit this in fear of this coming back to bite me in the butt is there anything i can do or anyone I can contact for help?

My daughter had the opposite problem, marked a No that should of been a Yes. When you print it off, mark out the previous answer (harder to "unmark", I understand that), then write the explanation in the notes section. Another option could be to call Concorde directly if you are looking for a way to completely erase marking yes, she didn't look into it any harder than reading the directions that said to print, change and explain.
