Please I need your assistance, I have been stuck on R251.02 remedial AMI. I keep getting the message below and no matter how the DD 2807-2 get corrected, it’s still come with the below AMI. Please I don’t know what to do or how else to correct it.
“DOES NOT REQUIRE A PHYSICIAN'S APPOINTMENT: Please complete the following
omitted ITEMS: missing provider comments for all positive answers on the ATTACHED REPORT
of MEDICAL HISTORY DD FORM 2807-2. Provide complete details (e.g., frequency, age at last
occurrence, severity, circumstances, etc.) for any "yes" or "don't know" responses.”
