Multiple Letters for Same DQ Code

So my DS's DoDMERB portal shows multiple letters sent for the same single DQ Code (Hearing). Is it because they have IT issue or because he applied to multiple SA's and DoDMERB only sent DQ to applicable SA? Is the review done and that why they displayed a status of "Single" DQ. In order of preference, DS applied to USNA, USCGA, USAFA, and USMA. USFA notified him of the DQ and a waiver is under review. We are hoping that he failed the standards for two and passed for his first choice. Thank in advance for any feedback.
 
Just got confirmation that it is not an IT issue. DoDMERB does send individual letters to the applied agency (SA's). Patiently waiting to see where the other DQ letter went.
 
It's possible the other SA hasn't requested the DoDMERB be done yet, so they may be unaware they need to send a letter for that SA. Or it could be something entirely different.
