If I have to make a wild guess based on DD experience, it looks the videos got rejected and hence CFA failed. Best bet would be to send email to RC and ask.
Your file probably went to the committee and they reviewed everything, including your CFA and they found you failed it. Email your RC and FFR to know for certain.
