According to the NROTC website: Commander Naval Service Training Command 2601A Paul Jones Street Great Lakes, IL 60088.
It should tell you in your application package. You can have a Navy Recruiter do your AFA and they will electronically send it it usually.

Tried all but getting multiple sources to send to... I have 3 NROTC coordinators who have not yet responded and am hesitant about mailing the packet.
 
Have you completed the entire application online? Once completed, my DS was contacted by his nrotc coordinator. All additional forms including the fitness test were sent directly to the coordinator via email.

Yes. I completed it and got nothing in return except for my coordinators email/phone number. Neither to which he replied.
 
It's possible the recruiter info they sent you is no longer valid. Try contacting your geographically appropriate Navy Recruiting District using the following link:


There is a Contact Us tab on the top right side of each NRD website. Call Admin or Operations and ask for the name/number of the District's NROTC coordinator.

DO NOT mail the AFA to NSTC in Great Lakes.
 
+1 to above. YOU cannot mail in the AFA, it has to come directly from the person administering the AFA, usually transmitted electronically (via email).
You could try and contact Your Coordinator, but when our son's did theirs, they did them thru local recruiters who transmitted the results themselves.
 
+1 to the above, my DS's PFT (NROTC-MO) was administered and submitted by the local USMC recruiter. Good luck.

Oh man, I mailed it out today to Great Lakes. Good thing I have a copy.
