NROTC request for transcript

All, there is a form required for the NROTC/Marine Option scholarship that is a request for a secondary school transcript It is clear that the top section should be completed and signed by the student, and the rest of the document is written in third person ("this applicant") and appears it should be completed and signed by a college counselor. However, our counselor is not so sure. Can anyone shed light? I have an email into our recruiter as well.

I am not sure if the applicant here is in high school or college. But whichever the case, the school counselor should be able to sign and also put their title under their name. The intent is to protect the privacy of the applicant's grades and the counselor signature shows that the school acknowledges the request.

Having said all of this, I hope someone on thr forum who has gone through this process can answer with more confidence than myself.
