I am asking for help, we are filling out paper work for Indoc at Great Lakes and are confused about page of the FHCCA form.

Under emergency contact it asks for Patient Employer and Spouse Employer ... are they asking for parents info or students info?

It next asks for patient type ... he is a recruit correct?

And finally sponsor info
Who is is sponsor?

We need to submit three docs by mail correct?

NROTC release
New student indoc

And email the uniform spreadsheet

Best bet is probably for your DS to direct the questions to his NROTC unit for the best response. Sponsor generally refers to the military member, who is the "sponsor" for his/her dependents, i.e. spouse and children.
 
DO NOT FILL OUT THE PATIENT EMPLOYER/SPOUSE EMPLOYER OR SPONSOR INFO. DS called and was just scolded for not following directions (although I've read through the directions as well and that is not implied or stated at any point...there is one section that says some things do not need to be filled out now...so maybe that's what she means?) and trying to fill it out. The PDF does not allow that part to be edited, so I'm guessing that's how we're supposed to know that it is not to be filled out. I don't want anyone else to be yelled at about this...

I don't trust filling the PDFs out. I do it all by hand because whenever I save a PDF I filled it out it always is blank. I did email my unit to make sure I did it right... Oops. I'll update when they get back to me.
 
My DS and I finished all his paperwork
He emailed his unit and they said parents for patient info. To be safe we are sending two copies with parents info for patient and one blank along with a note. If I hear anything Back I’ll post
