NY State Traveler Health Form for Incoming Hot-Spot Covid States:

The way I understand the form... the 2nd paragraph states: if you are traveling for less than 24 hours and/or passing through NY then 14 Day does not apply.
 
I think that means you can travel through Texas on your way to NY and not have to QT if you are not from a hot spot.

The completion of the questionnaires is voluntary, said health department spokeswoman Jill Montag.
"There are no penalties for passengers who don’t fill them out," she said.

I take it to mean that if you're from a designated (high COVID) state to NY, the form applies.
If you're travelling THRU a designated state (for less that 24 hrs. in that state) to NY, the form doesn't apply.

Either way... it looks like the form is voluntary... so far... :)
 
I agree the form is voluntary, but I will fill it out. It's an important public health tool. If I or someone sitting near me on the plane I fly in on later comes down with Covid, having this information will be a huge help to contact tracers. That may help prevent or mitigate an outbreak of cases, including among new cadets. So I will feel honor and duty bound to fill it out.
