Obtaining Form DD785 for AFROTC disenrollment

I was an AFROTC cadet from 2007-2008 and voluntarily disenrolled. I am now preparing an OTS application and AFRSI 36-2001 states that although I do not need a waiver since I was not contracted, I need a memo from the detachment and a completed Form DD785 (Record of Disenrollment from Officer Candidate-Type Training) in order to apply.

I was able to get a memo from the detachment verifying my dates of enrollment, but they have no other information. None of the original cadre from when I was a cadet are at the detachment any longer. AFRSI 36-2001 states that HQ AFROTC should be contacted if a DD785 cannot be obtained, but the number listed sends me to a voice mailbox no matter what time I call and my messages have not been answered. I have also left messages with the 800-number listed on the AFROTC website and used the contact form, but still haven't heard back from anyone.

Does anyone know how I could get in touch with HQ AFROTC without ruffling any feathers?

Secondly, it is my understanding that Form DD785 isn't a form that is necessarily filled out as soon as a cadet disenrolls and kept on file -- it seems to be a form that is partially filled out by the requester, then sent to the commissioning source agency and they complete the rest (circumstances of disenrollment, recommendation rating) each time they receive one and then send it back. I am still in contact with the former Commandant of Cadets at the time I was enrolled and he wrote one of my letters of recommendation. Would there be anything wrong with just having him complete the form?
 
Maybe talk to the recruiting flight commander currently at the det and asking them how to get into contact with HQ on the matter.
 
Update in case anyone is curious:

I managed to get in touch with HQ AFROTC and found out that disenrolled cadets only get Form DD785 if they're contracted cadets (including national scholarship recipients).
