Part III: Letters of Recommendation?

Where can I find a print out for my 3 references to fill out?

I printed out the application, Part III contains 2 pages. It looks like there is space for my high school counselor but what about the other teachers? Do I just make 2 or 3 more copies of this form? Or does this just require a written letter being sent from the individuals themselves without any instruction from the Academy?

I sent in the 1st part of my application about a month ago. I haven't received anything in the mail and I don't have a candidate login. This may be the problem but I'm not sure. I was under the impression that these forms would be available from the start.
 
When i came across this back in October i was confused also. I just told my teachers to give their letters of recommendation to the guidance department and guidance put them in the envelope with part III and my transcripts. A week later i received a letter from admissions stating that my application was complete so that is one way you could do it. Hope this was some help.
