Permit to Report Immunization Questionnaire.

Am I missing something, how do you fill out the top part? What's the requiring document? Do yo leave the top half blank and just fill out BMU and BHC?
 
We just left it all blank, and he can fill it in on I Day when/if he needs more shots. Someone on the Facebook Parents page called Admissions and that's what they said to do.
 
I took the form to my son's pediatrician and they made sure he had all of the required shots and they filled in the dates on certain things. Another parent recommended that we do the shots BEFORE I-day so that the Plebe has one less thing to adjust to during the strenuous physical activities.
 
I think he's talking about the other immunization form -- the questionnaire -- not the yellow form. The yellow form must be completed by medical provider, with one copy mailed in and two carried to I Day. But the other form was confusing, and we were given the guidance to just bring it to I Day.

We also tried to get what shots we could from our clinic, but they didn't have Bexsero. So, my son will have to get that on I Day. Some larger clinics may have it.
 
For the immunization questionnaire, we filled out the answers to the questions that will not change between now and I-day but left the rest blank to be filled out on I-day if my DS needs more vaccinations.

We couldn't get Bexsero at our clinic either so my DS will have to get that on I-day as well. :eek:

Yeah youre right, I was talking about the questionnaire. So you dont have a requiring document either? Who told you to just bring it in, the BGO?
 
The instructions say to complete and bring it with you on I Day. So, it's not one that you are supposed to mail in. This form was the most confusing, so there were a lot of questions about it on the 2019 Parents Facebook Page. One of the parents called admissions with questions about requiring documents, etc... They told her just to leave it blank and have her son bring it on I-Day.

I suppose you could complete some of the form if you like. You could complete your name, date of birth, age, and answer questions 2-14. The bottom is for the Immunization staff to use. You'll get your DOD ID # of I-day from what I understand. Question 1 is worded as "at this time" are you ill, so I'd leave that blank until I-day.
