I am not finding the NHCA immunization general consent (BMU and BHC)?? Any ideas where I can find it?

The only consent on immunization that DS has shown me immunization was at the top of the Immunization Record (green form?) where signatures of both the appointee and parent/guardian are requested.
 
DS and I were looking at Fleet Hometown News Release NAVSO 5721/1 Form. A quick question -- does this form have 1 or 2 pages? in his PRT only one page included but at the bottom it says Page 1 of 2.

A somewhat rude awakening I received when the PTR came to our house was when my son told me he was over 18 and didn't need my signature anymore. Ouch! Congrats on the appointment:smile:
