OK, DS's PFT scores were just marked as approved on the application, however the dashboard also still says "sent". We just e-mailed CC and asked if the form the coach should get is the exact same one that we have been sending in, and also informing them that on the dashboard it says "sent" even though on the application it says "approved".
We also asked about the guidance counselor form is the same as the one we can print out and hand deliver to guidance, and then submit, ourselves.
