Please help with missing PTR forms

DS is going through his PTR packet, and it appears a couple forms are missing --

Uniform Measurement form - Can we access this online? If so, does anyone know the link?
NHCA Immunization General Consent (BMU and BHC) - Is this just at the top of the Immunization Record, or a separate form? Again, how would he access it if it is?

DS did not receive the PPD form in the PTR package. Is there a form or just a record from his DRs office?

Is there a block for Date of PPD and Reaction on the Immunization Record form? Yes, there in the middle!
