Need I have multiple sources to get this form filled out? For example, going to multiple locations like Town police sheriff’s office, etc?

As per the instructions provided, you need to get the form filled out by every jurisdiction you fall under. For me, that was my County's Sheriff' and the City Police. To make things easier, bring envelopes (with postage) addressed to the Academy with the forms and CALL THE OFFICES. Make sure you go to the right office to release the information.

Our son did city police where he attends school, city police where he worked, city police where we live and county sheriff where we live. Provided pre addressed and postage applied envelopes to all.
Showed up as date received in his portal.
