Possible DQ?

My son is applying for USAFA/USN class of 2025 and had a minor L5 issue over 2 years ago which has resolved 100%. What type of documentation other than a letter from his spine specialist clearing him and radiology reports should be bring with him to the physical? Actual MRI film? Would love any input.
