DS is coming home from college this weekend to complete his entire PTR package. He will not be home again before the deadlines. For the PPD skin test that must be documented it says "form" on the checklist. Is there a specific "form" to be submitted, or is it just the square on the bottom of the gold shot record card?

Thanks Lukieboy06. He went today. By the way, the skin test requires a second visit 48 hours later to check for reactions, so any college kids coming home for the doctor visit need to be aware.I believe it is part of the yellow immunization form. There is a portion there for PPD.
