Pre-candidate Questionnaire

Can someone tell me what kind of information the academy asks for on the questionnaire?
 
Well, its pretty general stuff. It's all self reported. They generally like to know your name (obviously), all your basic general info (as if you were applying to a regular college). They also want self reported test scores, GPA, class rank, etc. You also fill out a kind of resume, i think. I'm not too sure, I filled that out such a long time ago.
 
The questions are not particularly hard, but they include information you may not have on hand.

The way the USAFA questionaire is set up, you have to enter one piece of information before advancing the page. Try to look at the Naval Academy Precandidate questionaire (also on the above link) where you can see all the questions at once. Questions you may not anticipate include the number in your class, class standing (ie 4 out of 140), Social Security #, and Congressional District number.

If you are first doing this for summer seminar, it asked you for a resume broken down into the following sections:

Academic Achievement
Athletic Achievement
Civil Involvement
Leadership Positions
Work/Job Experiences
Additional Information

Actually, I'm sure there were 5, so two of these were combined, but my son has reworked his resume a zillion times since then so I can't figure out the original. It'll give you somewhere to start. Incidentally, for WP and Navy SS's, it was much more fill in the bubbles yes/no with really nowhere to paste a resume.
