Prereq's for the actual app

So what is actually needed for the app? I wish to be ready when the application opens (I will be a junior this fall) and I want to be as best prepared as possible.

Will I need teacher recs? (how many and from who?)
What kind of essays?
Transcripts, CFA, etc.
