Does anybody know that these acronyms are? I asked my dad. He didn't even know. And we're not sure whether USAR stands for US Army retired or reserve. And the information in the parenthesis, is that the discharge order; DD214? Thank you

That form appears to be the Chronological Statement of Retirement Points from the US Army Reserve Personnel Center (just Googled it).
 
That's us army reserve. Print the form, sign it, attach the dd 214, and then scan a copy to your region tech.

Last question...

I am choosing to mail in the letter, and I assume I mail it straight to West Point admissions?
