Process for medical records

just wondering what the procedure is for providing medical records at dodmerb exam. I’ve never really had one doctor and just go to urgent care clinics if I’m really sick so I’m not really sure how I’m going to scrape up all medical records. If need be, I will make it happen but just didn’t know how the process went about.

only the forms they give you for your initial exam

You just need to fill out the paperwork that goes along with the exam. Later, for every yes you put down you will be asked to supply records for that. So you could start thinking along the timeframe and what clinic you may have been seen at. Think back to your early childhood years--you could be asked for that info.
