There is a prescription for polio in the Appointee Kit that you can print and take with you to your county health department or doctor
 
Yes for some reason that was initially denied. They acknowledged that I had the form they just said that they will follow the cdc recommendations.

I was able to go to a walk in clinic and show them the prescription and they did it for me.

The same happened to me. I had to go find another clinic because the first wasn’t willing to give the vaccine even after showing the prescription.
 
Just want to say even though I've said it before:

It has been an honor to get to know all of you who are joining the Class of 2026. While I won't be there, with less than a month until I-Day, I know from my interactions how incredible this class will be. I had the opportunity to go on FVE in December and meet 20 or so other outstanding young people (of which I am only one of only 3 I know of who didn't get an appointment if that can speak to their qualifications, talent, and dedication) that will join you in short time.

For the “Servicemembers’ Group Life Insurance Election and Certificate” form, do I put $0 as the current amount of SGLI? (I’ve been a civilian for all of my life)
 
One more question, is the reporting-in time and additional information supposed to appear in my portal after I submit the appointee kit? I don’t see anything new in my portal.
