R251.22 Paperwork

Hello!

I got my results from the DoDMERB back and I need to complete the R251.22 Administrative remedial which does not require a physicians appointment. My only question is how should I go about the paperwork? Is it acceptable to simply type up my response in a Word document and mail it? Or is there a certain format/form required for this response? Thanks!

As a remedial (R) code, it likely is only requesting copies of records for something that you reported on your original physical paperwork. The letter should tell you exactly what you need to send in. If you have any questions call DoDMERB at the number listed for the first letters of your last name as listed on their website. My DD had to deal with a remedial as well and a call to DoDMERB provided some outstanding and timely customer service. Of course, that was a few months ago when they weren't as busy as they likely are right now. Good Luck.
