The problem I found with that form is that the DATE SIGNED portion if filled out and cannot be emptied.

Same happened to me. I just printed out that form that you have a link to and filled it out and sent it in. The academy accepted it so it's fine.

I made a mistake as well. Fill out the online form, print it out, white out the date, and then photo-copy it. That leaves no trace of the white out.

The instructions they give you conflict with the sample that is given, I think that is what is so confusing...
