Remedial Appointment Question

I recently went through my optical and medical examinations for DoDMERB. My urinalysis showed traces of protein and I've been asked to retake it. I've scheduled a appointment and logged it in the DoDMETS applicant center. However, the only form it gave me was titled, "DoDMERB Remedial Appointment Notification."

The form is confusing as it states I must mail it to Concorde and respond with my appointment dates, but they are written right on the paper. Although, My main concern is that I don't have the right forms for the doctor. There's no instructions for the doctor on this notification and there's no other forms to print from the DoDMETS applicant center. Any advice would be appreciated.
 
My son had the same thing last year. He laid off all exercise a few days before the re test and thee was no problem.

i seem to reall the doctor office knew exactly what to do-just give them a ring and ask.

Good luck. Lay off a few days and forget about the protein shakes and you will be fine.

I had the same question like 4 days ago. However, the people at the remedial facility knew exactly what to do, I did not mail in the form and DodMETS now says case closed for me. So i think you're good either way.!
 
Being asked for a remeidal means that DoDMERB (for the initial determination) and the waiver authority (for waiver decisions) feels they don't have sufficent data to basewd their decisions on. Hence, they ask for more "stuff.":thumb:
 
Thank you for the responses. It's a good thing I did call Concorde as the receptionists at the doctors asked for documents. Luckily Concorde was able to fax them the information earlier that morning.

My DoDMET status is now "case closed." Am I supposed to send Mr.Mullen any information? That seems to be the case from other threads I have read here. Or do I give Concorde 30days to send the info first?
