Hello Internet,

After being recently reviewed for my DODMERB exam, results came back as remedial for ear wax removal. Is there a specific form that I have to hand my physician to fill out and then send that to DODMERB or does the physician send his personal forms to DODMERB?

And How long does this complete process take?
 
I had the same remedial code. You should be able to download the letter that contains all the codes. Simply take this letter to your physician (you will have to pay for the remedial). Your doctor will remove the wax and then check to make there are no injuries to your ear drum.

Just make sure to carefully read the letter that I mentioned (it should be on your DoDMERB portal) and follow any specific instructions.

So after he does the checkup, does he submit his own form to DODMERB (and how would he do that), or does he give the form to me and then I mail/fax/email it to DODMERB?
 
My DS submitted all his remedial forms to his assigned DoDMERB technician. He scanned and emailed them.

I asked the doctor to provide me with the medical report. Then I scanned the report and emailed it to DoDMERB. Make sure to follow up with DoDMERB to ensure they got the AMI (additional medical information) since the email I sent them to was no longer in use.
 
After getting the "issue" fixed by his physician, get a letter or treatment note from the physician and simply upload it into Dodmerb. Pretty easy, but make sure all issues are addressed.
