Hello! I am currently going to the phase of getting all my records to send. I was going through my pharmacy records and found a medication that I have been prescribed and filled but never ended up taking. Because of this I forgot it even had existed and never included it on my questionnaires. How could I go about notifying them and noting that it was never used? Thank you!

Send an email to your case manager.

Another option is to just wait and see what DoDMERB says. They may gloss over it and won’t ask for additional info. They may think it’s worth a deep dive and may ask for additional info. At that point, tell them you never used it. If you answer that way, then there’s a chance DoDMERB may ask you to give the notes from the doctor visit in which the med was prescribed which would tell why you went it to the doctor and what the doctor thinks you had causing the med to be prescribed.

I went through a phase post-covid where I had some anxiety. I took a regular medication for 7 months, which I reported. They prescribed a second (different) medication in case I had an anxiety attack. I was off the first medication fast, and never took the second one.

For that, just send an email to your case manager. The case manager list is on the DoDMERB website and applicants are assigned by last name.

The case manager may accept the email as is or may ask you to provide a signed and dated pdf memo. Either way, you’re at least letting the case manager know you never took the med that was prescribed.
