I just scheduled my medical remedial with the provider Concorde told me to, after calling two other places and having them turn me down. The thing is, I don't know if there is any other paperwork that the doctor needs. The only form I printed off of the Concorde site was the appointment reminder form.

If you are using the provider Concorde referred you to, they (concorde) will fax the paperwork to them before your appointment.

That's what I thought before, but the supposedly faxed paperwork to the first provider was never received, and now I'm afraid that the new provider won't get the paperwork either. No one in the office I called even knew what DoDMERB, Concorde, or ROTC was.

I would suggest you call Concorde, there are numbers on the letter you can use. Ask them to make sure the provider gets the paperwork they need for your remedial exams. I found Concorde to be very efficient with that and with helping us find a provider who would see our son right away. The ladies we spoke with at Concorde were very helpful. All the best!!

I will concur with pmlutton. Give Concorde a call, they will make sure all goes as planned.

Ok, I will call first thing tomorrow. Thanks for your help. Sorry if I seem a little worrisome it's the last thing I have to do.
