ROTC Nomination to West Point

I got in the mail a "Recommendation for ROTC Nomination to West Point" form in the mail because I will be applying for one. I was hoping someone could tell me who I am supposed to have fill out the form. I would think it would be my Military Science 101 Professor but would like to know what other people think before I contact West Point about it. Thank you.

I think your accurate answer is going to come from West Point. Although this is a great place for discussion, I do not understand why folks are reluctant to ask the people whose job it is for accurate information. I've heard the Minority Outreach Program. What exactly does it do?
