School conduct record

I know there's a police record check and all. But as for school conduct (suspensions, in school punishments), does the academy have a form i am missing for school conduct? Or will admissions or someone of that degree do a follow up in late june?

to supplement for this, I simply obtained a letter from my Vice Principal that outlined my conduct and behavior. Im just beleaguered as to if im missing a form for this or if the academy will conduct their own follow up on school behavior.

If I recall correctly from last year, the record form was intended to be copied and supplied to all sources that needed to respond.
