Security form question

Hi,

Quick question. My son has been diligently filling out the 172 page security form. It asks about his parents (us) and former names. I have gone by a nickname since I was born. When the security questionnaire asks about prior names, does that get listed and how?
Should he specify 2 entries, one for my nickname with maiden name and then one for my nickname and married name?

What complicates this matter is that when some research papers were published, my manager included me with my nickname and maiden name on some of them, and with my real name and maiden name on others. Otherwise, I have never been "officially" known or registered by my nickname.

Would his Admissions Officer be the best person to contact on this?

I'm sure if you list all the names you have gone by in the past, everything will be fine unless you used those names for fraudulent purposes such as to conceal your true identity. If you have 30 AKA's (also know as) names, that might raise suspicion as well.
 
I'm going through my own security update. Write down every name you have ever gone by.
